form work on your server is not within the scope of the support offered with the template.

This is a "software specific" component -- it is not within the scope of our support to teach you how to use Dreamweaver or advanced scripting methods to have form results sent to you
All problems that occur with making a form developed in Dreamweaver are normally server end issues
Having said that, we do want to get you started and on the right track.
How to edit the form...
If you are using Dreamweaver to edit your website, you should know there is no "magical" method of making a form and having the results sent to you, like you can do with FrontPage.
Most web hosting providers include a "form handler" method that you can point your form to. The first thing you should do is contact your web host to see if one exists.
There are many sources where you can learn how to edit/create/modify forms with Dreamweaver. Here are some links to outside websites which cover that sort of material:
